Fenway Health strives to deliver the highest quality care and services to all of our patients and community members. In recognition of that commitment, we are pleased to share that we have been awarded a quality improvement grant of $206,254 by the U.S. Department of Health and Human Services (HHS), as part of the department’s 2017 Quality Improvement Awards.
This monetary award represents a culmination of multiple achievements across a number of the award categories, including being named a “National Quality Leader.” It is also the largest of awards given out to any Federally Qualified Health Centers (FQHC) in the state of Massachusetts this year!
Counting among an overall total of 1,333 health centers awarded across the U.S. states, territories and the District of Columbia, we are being recognized with these funds thanks to our high performance within a total of six out of an overall eight categories making up the awards process. The funds will be used to further quality, efficiency, and effectiveness here at Fenway Health, so we may continue to deliver the highest quality of care and services to the patients and community members whom we serve.
The categories for which we are being recognized, having demonstrated achievements to HHS through our Annual Data System Report (UDS) for 2016, include:
- Electronic Health Record (EHR) Reporters
Employed EHRs to report on all clinical quality measures (CQM) data for all of the health center’s patients.
- Health Center Quality Leaders
Achieved the best overall clinical performance among all health centers.
- National Quality Leaders
Exceeded national clinical quality benchmarks, including Healthy People 2020 goals, for chronic disease management, preventive care, and perinatal/prenatal care.
- Enhancing Access to Care
Increased the total number of patients served and the number of patients receiving comprehensive services between 2015 and 2016.
- Achieving PCMH Recognition
Awarded to health centers whom received PCMH recognition for their service delivery sites.
- Addressing Health Disparities
Met or exceeded Healthy People 2020 goals by each race/ethnic group or made marked advances in moving each race/ethnic group towards the Healthy People 2020 goals.
