Fenway Health is participating in an exciting collaborative with other health centers, hospitals, and community based organizations across the city whose goal is to meaningfully address the health needs of Boston. The health needs assessment will drive priorities, funding, and cross-agency initiatives to address some of the most persistent health gaps and challenges we face in unique ways, and will develop a city-wide community health improvement plan.
It is critical that this assessment capture the needs of underrepresented communities, including LGBTQ folks and people living with HIV, and will have a particular focus on racial inequities in health outcomes.
If you live in Boston, we encourage you to take a few minutes to fill out the survey. It is open to all Boston residents, and is online here: https://www.surveymonkey.com/r/bostonchna. The survey is available through the above link in English, Spanish, Portuguese, Vietnamese, Arabic, and is also available in Haitian Creole through this link: https://fr.surveymonkey.com/r/BostonCHNA-HaitianCreole.
You can help ensure that Boston has a city-wide health plan that reflects our diverse communities – please consider taking this important survey today!
