To participate in The LEGACY Project, talk to your medical provider at your next appointment. You can take the survey at your appointment, or it can be emailed to you to complete at a later time. Everyone will take 3 surveys over the course of one year. After you complete each survey, you will be entered into a raffle to win a gift card!
